First Visit

Your initial appointment is designed to provide a comprehensive evaluation and a clear understanding of your treatment options. In many cases, treatment can begin the same day as your consultation. However, patients with more complex medical histories or treatment needs may require a follow-up visit to ensure the highest standard of care.

To help us serve you efficiently, please bring the following to your appointment:

• Any recent dental X-rays from a previous provider, if available
• A current list of medications you are taking
• Completed dental insurance forms, if applicable, to assist with claim processing

Important: Patients under the age of 18 must be accompanied by a parent or legal guardian.

Please inform our office in advance if you have any medical conditions that may affect your care—such as diabetes, high blood pressure, artificial heart valves or joints, or a history of rheumatic fever—or if you are currently taking medications including heart medications, aspirin, or anticoagulants.

X-Rays

If your previous dentist or physician has taken recent X-rays, you may request that they be forwarded to our office. If time does not permit, please obtain a copy and bring it with you to your appointment. Should additional imaging be required, it can be performed conveniently at our office.
